A Release of Liability form must be signed and submitted for each child and adult participating in the Firefighters Children's Parade.
RELEASE OF LIABILITY
The undersigned is a participant in the 6th Annual Firefighters Children's Parade on Sunday, February 14, 2010. In consideration of participating in such parade, the undersigned hereby agrees to assume all responsibility and liability for any and all damage, personal injury, or other loss of any nature and further releases and agrees to hold harmless the Firefighters Children's Parade Krewe and its agents from any and all liability, claims, damages, and causes of action of any nature related to said parade.
The undersigned hereby acknowledges and certifies that he or she has read the Children's Parade Float Rules, the Official Parade Information Form, and this Release of Liability, fully understands them, and by signing below agrees to all terms and conditions included therein.
